Motorcycle licensing laws

I wonder how licensing requirements differ from country to country for obtaining a license to drive a motorcycle.  What is the age minimum in your country? Is there both a book test and a driver skills test? Is there anyone forbidden from trying to obtain a license who is at least the minimum age?
